Booking your Appointment

• You will receive a confirmation email when you have completed your form. Directions are sent out with this form, please read.

• If you have had your claim registered elsewhere please bring it in, so you are not charged an administration fee.

• If you would like a call-out appointment or an after-hours appointment feel free to either call or email and we can work out a time to alleviate your issue.  

• If your ACC number is older than one year, there will be an additional administration fee charged to you to do an ACC32.
